Newborn Checklist
  • Completed New Patient Intake Form prior to the visit.
  • Any paperwork from the hospital, including hearing screen results and immunization records.
  • Any available insurance information.

       (often only the parent’s insurance is available at the first visit).

  • Plenty of diapers, an extra set of clothes, and a blanket to wrap baby in during the visit.

Before Your First Appointment

A few weeks before delivery, call the office to let us know you are expecting.


You may want to fill out our New Patient Intake Form ahead of time.


When the happy day has arrived, you can tell the hospital staff that you have chosen us as your pediatric office and provide them with our information.
