We understand that navigating an ADHD evaluation can feel overwhelming, and we’re here to support you with expertise and compassion. Our office provides comprehensive, evidence-based evaluations for both new and existing patients in a structured and thoughtful manner.


Our process includes an in-depth parent consultation, a thorough review of standardized rating scales from caregivers and teachers, and a developmental assessment to understand your child’s unique strengths and challenges. Based on these insights, we create personalized treatment plans, which may include behavioral strategies, school accommodations, therapy referrals, and, when appropriate, carefully managed medication.

 This process requires the parents to commit to a contract to complete the evaluation. Children with complicated emotional and mental health issues may be referred to a child and adolescent psychiatrist as well.

Initial Evaluation Checklist

  • Signed contract for corresponding provider
  • 1 Parent/Guardian Vanderbilt form per Guardian
  • 1 Teacher/Counselor form per each Core Curriculum Teacher
  • Elementary School (Typically 1-3 Teacher forms)
  • Middle School (Typically 3-4 Teacher forms)
  • High School (Typically 3-4 Teacher forms)
  • Email completed forms to ADHDEVALS@PEDIATRICSATB.COM
